Celebrating the impact of our Reablement Team

We are proud to celebrate the incredible work of our Reablement Team, who help people regain their independence after illness, injury or a hospital stay.

Reablement focuses on supporting people to do things for themselves again, from managing daily routines and getting around safely to building confidence at home.

By setting practical goals and providing encouragement, advice and hands-on support, the team helps people rebuild skills and maintain their independence.

This short-term service can make a significant difference, helping people return home sooner, avoid unnecessary hospital admissions and reduce the need for ongoing care. 

We would like to thank the team for the compassion, dedication and support they provide every day.

Reablement in 2025/26 so far:

  • 1,606 people supported
  • 924 referrals received through hospital discharge
  • 682 referrals received through community and health services, including Discharge to Assess, Health Community Response Service, Emergency Duty Team, Gateway and Social Work teams
  • 1,089 people completed reablement with no need for additional support
  • 69% of people reported an improvement in their overall wellbeing following reablement support

These figures highlight the positive impact the team continues to have on the lives of local people and their families.
